How the 1–5 significance rating works
5Practice-defining — Resets the standard of care for something you do every week.
4Changes practice — A new first choice, a de-adoption, or a guideline that rewrites how you plan a case.
3Changes some calls — Sharpens a real but narrower decision — matters when that patient is in the chair.
2Worth knowing — Confirmatory, incremental, or narrow. Read it; your protocol probably stays the same.
1Minor update — Terminology or administrative.

The rating answers one question: how much of everyday practice has to change because of this paper. It is deliberately harsh — most good papers are a 3, and a downgrade for a weak or fragile study is always explained on the page.
